Our Periodontal team is dedicated to providing exceptional periodontal care and services. Our priority is your dental well-being and we take every precaution to ensure your treatment is conducted responsibly. To help with the investment in your dental health, we have provided details on our payment policy.

Payment of services is expected to be paid at time of service. We accept Cash, Card (all cards are accept) & Check.          

Insurance Information

We will gladly submit your primary insurance claim form if you have dental insurance.

Your initial exam/consult fee is payable by you, and we will file a claim with your insurance for reimbursement. A pre-determination can be sent to your insurance to determine the amount they will reimburse you, but you will be responsible for the full payment.

It's important to note that as a dental care provider, our relationship is directly with you and not your insurance company. Filing insurance claims is a service we offer, but ultimately the responsibility for payment of all charges falls on you from the date of the services rendered.

Additional Terms

If you decide to cancel the appointment, we would appreciate at least a 48 hours cancellation notice.
